Status:

Found single or scattered on ground in oak-hickory forests.

Description:

Cap: Dingy white/gray; convex to slightly depressed, covered with gray warts; margin usually with veil fragments; flesh whitish to pale gray. Gills: white, crowded. Stalk: White above, grayer below with concentric scales near base; white delicate ring is often missing; spindle-shaped bulb (J. Solem, pers. comm.).
